MassHousing Get the Lead Out Application
The 13 items listed below comprise an application to the MassHousing Get the Lead Out program.  If you need assistance with any of these items, please contact Alice Davey by e-mail adavey@palmer.lib.ma.us ; by phone 283-2614; or in person at 1029 Central Street, Palmer.

1.      Print and complete Fannie Mae Form 1003
2.      Attach a copy of the lead inspection report
3.      Attach a copy of the Deleader Contractor License issued by the Division of Occupational Safety of the contractor you wish to hire
4.      Attach copies of all estimates you received, sign the estimate from the contractor you wish to hire.
5.       Print, complete and attach IRS form 4506-T
6.      Attach a copy of all mortgage documents secured by the property to be deleaded
7.      Attach a copy of the deed for the property to be deleaded
8.      Attach copies of pay stubs for the last 8 weeks
9.      Attach copies of your federal tax returns for the last 2 years
10.     Attach copies of paid receipts for the most recent property tax payment, water payment, and sewer payment.
11.     Attach a letter stating your intent to continue to occupy the property
12.     Attach a copy of homeowners insurance binder
13.     Print, sign, and attach the Residential Deleading Advisory
